Continence assessments are funded under Capacity Building, Improved Daily Living. Nursing services sit under NDIS 0114. Personal care sits under 0107, dementia support under 0104, domestic assistance under 0120, and community participation under 0125.
We also hold NDIS registration for high-intensity daily personal activities, including complex wound management, catheter management, enteral feeding, tracheostomy and ventilator management, subcutaneous injections and severe dysphagia management. Registered support classes: 0104, 0107, 0108, 0114, 0116, 0117, 0120, 0125.

Health Focus Nursing visits participants across metropolitan Melbourne and the Geelong region. Melbourne coverage includes the Maribyrnong, Brimbank, Melton, Hobsons Bay, Wyndham, Moonee Valley, Hume and Merri-Bek council areas, taking in suburbs such as Footscray, Sunshine, Werribee, Caroline Springs, Yarraville, St Albans, Williamstown, Brunswick and Preston. Geelong coverage includes Geelong, Geelong West, Newtown, Belmont, Highton, Corio and surrounding suburbs.

Yes. Nursing care delivered in a participant's home is funded under NDIS support item 0114, Community Nursing Care. Participants with relevant funding in their plan can access it whether their plan is self-managed, plan-managed or NDIA-managed. Health Focus Nursing bills at current NDIS Price Guide rates, so eligible participants with available funding in the right support category have no out-of-pocket cost.

An NDIS continence assessment is funded under Capacity Building, Improved Daily Living, and is billed at current NDIS Price Guide rates. Participants with that funding available in their plan pay nothing out of pocket. The assessment is available across self-managed, plan-managed and NDIA-managed plans, and Health Focus Nursing invoices against the relevant support item rather than charging participants directly.

Health Focus Nursing currently has no waitlist and responds to all enquiries within one business day. Referrals from support coordinators are acknowledged within one business day and service agreements are issued within 48 hours. Visit scheduling then depends on the service required and your availability, and urgent nursing needs are accommodated where possible.

No. A GP referral is not required for NDIS-funded services. Any participant with an active NDIS plan that includes the relevant support category can contact Health Focus Nursing directly. Support coordinators, plan managers and family members can also make a referral on a participant's behalf without involving a GP.

Community nursing covers clinical tasks that must be performed or supervised by a registered nurse, such as wound care, catheter management, medication administration and enteral feeding, and is funded under NDIS item 0114. Personal care covers non-clinical help with daily living, such as showering, dressing, grooming and meal preparation, is delivered by support workers, and is funded under NDIS item 0107. Many participants receive both.

Yes. Health Focus Nursing is registered to deliver high-intensity daily personal activities, which require advanced clinical training and registered nurse oversight. These include complex wound management, urinary catheter management covering in-dwelling, in-out and suprapubic catheters, complex bowel care, enteral feeding via naso-gastric, jejunum and duodenum routes, tracheostomy management, ventilator management, subcutaneous injections and severe dysphagia management.

Yes. Participants can request a particular support worker and can ask for a different one at any time without needing to justify the request. Health Focus Nursing matches workers to participants based on needs, preferences and personality, and rosters for continuity so participants see the same faces rather than explaining their routine to someone new each visit.

Yes. Health Focus Nursing can provide support in languages other than English and works to match participants with workers who speak their preferred language. Language needs are discussed at the initial consultation so that the right worker is rostered from the first visit.
