NDIS Continence Assessment

A registered nurse comes to you, talks through what is actually going on, and writes the formal report your planner needs. Most of our reports are accepted the first time they are submitted.

Community Nursing

Catheter care, wound management, medication management, bowel and stoma care, right through to tracheostomy and ventilator support. Delivered by AHPRA-registered nurses, not general support workers.

Personal Care and Daily Support

Help with the parts of the day that are hardest to manage alone. hygiene, grooming, meal prep, getting to appointments and out into the community. We try to keep the same faces coming back.

Dementia, Palliative and Respite Care

Behavioural support and memory engagement for people living with dementia, comfort-focused palliative care at home, and respite for the family carrying the load day to day.

Everything else we do

Domestic assistance, social and community participation, companion care, diabetes management, overnight nursing and hospital-to-home transition care are all part of what we offer too.

Continence assessments are funded under Capacity Building, Improved Daily Living. Nursing services sit under NDIS 0114. Personal care sits under 0107, dementia support under 0104, domestic assistance under 0120, and community participation under 0125.

We also hold NDIS registration for high-intensity daily personal activities, including complex wound management, catheter management, enteral feeding, tracheostomy and ventilator management, subcutaneous injections and severe dysphagia management. Registered support classes: 0104, 0107, 0108, 0114, 0116, 0117, 0120, 0125.

Health Focus Nursing was started by a registered nurse who was tired of seeing NDIS participants passed between providers who never quite followed through. We are a small, women-owned practice, and we are LGBTQ+ friendly by design, not as a marketing line.

We serve Melbourne, Geelong and the suburbs in between. Our nurses hold AHPRA registration, our support workers are matched to you specifically, and if a plan changes, we change with it.
